Director of Practice Integration, Health Equity – Remote

Reporting to the VP of Health Equity, the Director of Practice Integration, Health Equity will have a pivotal role in building and executing system-wide strategies to strengthen Aledade’s culture of health equity. This vital leader will facilitate best practices in fostering an affirming and inclusive culture where all practices have an opportunity to succeed and create a patient care environment where they can achieve their best health. The Director will serve as the practice integration and health equity subject matter expert who will work collaboratively with strategic partners across each Aledade market.
The Director will assess our practices’ health equity climate and current programs/initiatives, help stakeholders articulate their equity aspirations, as well as identify and recommend strategic and programmatic solutions to advance Aledade’s agenda on health equity excellence.
We are looking for individuals who genuinely believe that Aledade’s model of physician-led ACOs can improve healthcare quality and lower costs. This role can be based out of Bethesda, MD or work remotely within the US.

Duties include:

  • Support development and achievement of Health Equity Team’s goals 
  • Collaborate with Clinical Innovations team to drive implementation of hypertension pilots and develop strategic framework for future pilots to address health disparities
  • Develop an organizational strategy for health disparities reduction including the development and achievement of key performance indicators 
  • Collaborate with the People Team on staff training and initiatives to increase Aledade staff competency and capacity for supporting practices in addressing health disparities
  • Institute trainings for practice partners to better embrace and achieve health equity
  • Identify areas of need and design impactful initiatives to support success for minority practices
  • Qualifications:

  • Bachelor’s Degree required
  • Masters preferred 6+ years of progressive experience in practice transformation
  • Deep understanding of value based care, and demonstrated success generating buy-in and engagement with internal and external stakeholders
  • Demonstrated ability to work effectively with diverse populations.
  • Strong leadership skills and coalition building
  • Demonstrated effectiveness in organizational development and change
  • Strong interpersonal, communication, and presentation skills
  • Leadership experience in a complex start-up organization or health system preferred
  • Knowledge of events planning and marketing
  • Proficiency use of multimedia tools
