Health Equity Grants Specialist, Remote

As a key member of Aledade’s Health Equity (HE) team, the Grants Specialist will focus on supporting success in achieving health equity through Community Health Center (CHC) Engagement and specific health disparities reduction initiatives.  This role reports directly to the Senior Manager of CHC Engagement. The specialist will collaborate with Aledade Product and Tech teams to identify tech enablement opportunities and App enhancements that will align more nimbly with CHC grant program interventions. The specialist will identify targeted patient health outcomes for health center patients and work across teams to strategize a partnership solution. The specialist will engage with both field teams and CHC partners to develop alignment for grant opportunities, and to facilitate communication for proposals with both CHC partners and potential investors. The specialist will play a role in driving success toward the HE team’s defined objectives and key results. The specialist must be service-oriented, well-versed in (or eager to learn about) health equity, health disparities, CHC terminology and values, and value-based care. This role requires strong attention to detail, great communication skills (both oral and written), competency and adaptability with business intelligence and communications tools, and above all, a passion for health equity.  
We are looking for individuals who genuinely believe that Aledade’s model of physician-led ACOs can improve healthcare quality and lower costs. This role can be based out of Bethesda, MD or work remotely within the US.

Duties and Responsibilities

  • Support the HE team in research of specific health disparities to target for intervention within CHCs
  • Develop grant application template, including Aledade boilerplate and proposal for intervention for various health disparities 
  • Research and identify large investor opportunities aligned with selected health disparities goals
  • Collaborate with field teams and BI to identify CHCs as ideal candidates for new proposals 
  • Collaborate with Product and Tech to develop roadmap for App solutions which will allow CHCs to adopt better workflows for grant performance 
  • Collaborate with IA to establish tracking and monitoring process for CHCs under new grant performance 
  • Facilitate communications amongst key stakeholders, both internally and externallyPrepare, analyze, and summarize various weekly, monthly, and periodic operational discussions, decisions, and results related to health equity for use by various key stakeholders.  
  • Develop actionable roadmaps for workflows and processes and decision trees for identifying and executing upon new opportunities 
  • Participate in trainings on health equity and be a champion of health equity within Aledade
  • Actively contribute to the team with ideas and talents
  • Qualifications

  • Bachelor’s degree or relevant work experience in lieu of educational requirements may be accepted
  • 4+ years work experience in healthcare or related field is required
  • Experience in project management
  • Experience with Google business suite, Tableau, and Salesforce CRM as data, communications, and project management tools is preferred. 
  • Comfortable tackling ambiguous problems with a high level of curiosity and autonomy
  • Highly organized and proactive in raising questions around clarity, prioritization, blockers, and other points to drive assigned tasks forward.
